Attention Grades 6 - 12: Athletic and Marching Band Physicals Required
Students in grades 6–12 who plan to participate in athletics and/or marching band must have a current physical examination form on file before participation.
The required physical form is available on the OSSAA website or by using the link provided.

Completed forms, signed by both the healthcare provider and parent/guardian, may be submitted to:
- The High School Office
- The student's coach
- The band director
Please submit all required paperwork as soon as possible to ensure eligibility for participation.
