Updated OSSAA Eligibility Process for All Student Athletes
IMPORTANT: UPDATED OSSAA ELIGIBILITY PROCESS FOR ALL STUDENT ATHLETES!
The OSSAA has transitioned all student-athlete eligibility forms to an online platform called Rank One. All students in either athletics or band will need to go through the Rank One process. In order for your son or daughter to be eligible to participate in athletics this school year, both OSSAA Eligibility Forms must be completed through the Rank One system.
If your student is a returning Sperry student, the process should take only a few minutes.
If you are a new student to Sperry, complete the forms, and the forms will be sent to your previous school for release. The athletic department will monitor the process to ensure all new student forms are signed by the previous school and will follow up with any additional questions.

Getting Started
- Click the link above.
- If you are a first-time user, create an account using your email address and a password. You may also choose one of the alternative login options (Google, etc.).
- Enter your student’s last name and Student ID number.
Finding Your Student ID Number
Your student’s ID number can be found in one of two places:
- Through the Parent Portal
- You may need to contact Administration for access
- Returning students will know this as their lunch number
Complete OSSAA Eligibility Form 1 & 2
When you return to the Forms Management home page, both OSSAA Eligibility Form 1 and OSSAA Eligibility Form 2 should appear in green.
- Green = Eligibility forms are complete.
- Yellow = The form requires review by the Athletic Department.
If additional information is needed, we will contact you. Otherwise, once the forms have been reviewed and approved, your student will be eligible for participation.
Other local forms that your student will need to complete are also available on Rank One as well.
